Our advances with deep plane techniques give us control of the breast height, width, and shape, and allow us to leave minimal scars. The lift can be enhanced with techniques to add volume, such as fat grafting and implant placement, if desired. This allows us to take any type of breast that may need a lift, whether it be low, hollowed on the top, saggy, or flat, and transform it to your new, favorite shape and size. Most women want a younger, higher version of the breasts they have now. This is best achieved with a vertical lift. A vertical lift puts the low, saggy breast tissue back up top, lifting the breast internally. The result is a younger version of your breast, before it aged, with a fuller upper pole (the top half of the breast), no hanging of the skin onto your abdomen, and a rounder profile. Incisions for this procedure are usually called a “lollipop,” where the scar goes around your areola and then vertically between your nipple and the natural crease under your breast.
For some breasts, younger and higher just isn’t enough. We need a total transformation. Breasts that are flat, stretched out, extremely low, or just were never that great, sometimes need a pocket lift. Created by Dr. Cassileth, the pocket lift takes breast tissue and places it under the pectoral fascia, a thin, but strong layer that covers the chest muscle. This allows for the breast to be imaged normally when you have an ultrasound or mammogram, but it does not allow for the breasts tissue in the pocket to sag. That means that even stretched-out breast tissue with stretched-out skin can be placed high, and stay high. This technique also works well for patients who have been told that they need a breast augmentation for upper-pole fullness, even when their breasts are already big enough. The incision options vary from a small scar just around the areola ("periareolar"), to a lollipop lift, to a full anchor (or “wise”) pattern, depending on how much extra skin needs to be removed. Since the breast lift is internal and supported by the pocket, the skin is not used to support the breast. Because the breast lift is internal, the “pocket” does all the work, supporting the breast, and putting no stress on the skin. This makes for thinner scars, and long-lasting results.
It is common for breasts to lose volume after breastfeeding, as the active gland takes over and atrophies the natural fat1. If you want to replace that lost volume, we can do that at the same time as the mastopexy. With our vertical lift technique, an implant can be placed either under the muscle or under the fascia to restore any lost fullness. For smaller increases in size, fat grafting can be combined with both the vertical lift and the pocket lift to increase the breast size by around one cup size. For fat grafting, you will also need to choose a donor site, as the fat will liposuctioned from that area and then purified and carefully injected into the breast. Patients sometimes have just the right breasts for a no-scar lift (where the scar is in the inframammary crease) or a circumareolar lift (where the scar is around the areola). These breasts usually have a good or slightly low nipple position, and the addition of volume or raising of the breast tissue restores the breasts’ aesthetics.
Mastopexy is well tolerated by most patients, and most patients take one week off work to recover. After the first week, your day-to-day activities should be relatively normal, although there heavy lifting and high-impact exercise are not permitted for one month. Large, or extremely droopy breasts sometimes require a two-week recovery, as more tissue is repaired and deeper work may be required. Drains are rarely used. We often recommend you stay overnight in our aftercare facility in order to receive the highest level of post-operative care. Incisions are carefully monitored and we have a one-year “scar guarantee” with free scar treatments if they are ever necessary.
